AMENDMENT TO THE ENHANCED CARE MANAGEMENT PROVIDER SERVICES AGREEMENT BETWEEN PARTNERSHIP HEALTHPLAN OF CALIFORNIA AND LASSEN COUNTY
SUBJECT: Amendment to the Enhanced Care Management (ECM) Provider Services Agreement between Partnership HealthPlan of California and Lassen County.
FISCAL IMPACT: This is a revenue agreement for Behavioral Health Fund/Budget Unit No. 110-0751.
ACTION REQUESTED: 1) Approve the Amendment with Partnership HealthPlan of California; and 2) authorize the County Administrative Officer to execute the amendment.
